We have a new opening for a Registered Nurse - Case Manager at Randolph Air Force Base in San Antonio, TX.
Core Duties: The duties include, but are not limited to the following; Interact with providers, USAF units, military treatment facilities, certification/waiver authorities, and other stakeholders.
Conduct case management, as defined by the Case Management Society of America (CMSA), Standards of Practice for Case Management & Defense Health Agency Procedural Instruction (DHAPI) 6025.2
Medical management; Continuously engage patient members, their military organization (their unit) & other stakeholders, regarding medical records, authorizations, approvals, care/treatment, care/treatment-plans & appointments, supporting services, and health-evaluations to ensure effective care and effective case-management for the service member.
