Liquid Personnel is recruiting Social Worker for its client's Learning Disabilities Team at Middlesbrough.
What will your responsibilities be?
Conduct strengths-based reviews of adults with learning disabilities living in Independent Supported Living (ISL) schemes, ensuring support arrangements promote independence, wellbeing, choice and positive outcomes.
Complete person-centred assessments and reviews, working closely with individuals, families, providers and the Accommodation Lead to evaluate support needs, identify risks and ensure placements remain appropriate.
Maintain accurate case records and review documentation, undertake home visits as required, and contribute to care planning and decision-making in line with Care Act duties, safeguarding procedures and local authority policies.
