LAYIQ - Registered Nurse - Diabetes Education Program at Texas A&M University System

Diabetes Education/ Care Instructions
Develops, maintains, and provides individualized diabetes self-education and support/care plans for program participants (patients) and monitors progress on a quarterly basis.
Collaborates with referring providers and other healthcare professionals who are not part of the Diabetes Self-Management Education and Support (DSMES) service regarding education provided, education plans, and outcomes.
Provides preventive education to the community and students.
Documents and maintains class participants’ education records and health care needs.
Maintains State Department of Health records as required.
Travels to various education locations and assumes responsibility of assigned location(s).
Patient Assessments
Performs comprehensive initial and quarterly reassessments of participant’s education and health care needs, including diabetes self-management knowledge, skills, and readiness for behavioral/lifestyle change.
Collects biometric data, including blood pressure, heart rate, weight, BMI, body fat percentage, blood glucose, and hemoglobin A1c (HBA1c).
Maintains and operates biometric equipment for initial and quarterly reassessments.
Documents assessment and biometric data.
House Bill 984 Training/ Other
Provides training to school personnel who are not licensed healthcare professionals in the care of elementary and secondary school students with diabetes.
Participates in continuing education activities to maintain licensure/certification(s).
May provide supervision for support staff.
Performs other duties as assigned.
