We are hiring a dedicated Outpatient Behavioral Health Clinician to join our team and provide direct mental health support to individuals in our community.
Provide group therapy, discharge planning, treatment planning, and assessments.
Complete mental health assessments on all patients to determine social service needs, diagnosis and plan of care.
Evaluate patient data and develops and implements a plan of care for each patient.
Work cooperatively as a member of the interdisciplinary treatment team.
Complete progress notes in a timely manner.
Utilize established resources to assist in completing case management duties independently.
Document treatment provided to patients in their medical record based on established case management guidelines.
