What will your responsibilities be?
Undertaking Care Act assessments, reviews, reassessments, and support planning to identify eligible needs and achieve positive outcomes.
Providing high-quality information, advice, and guidance relating to care, support, and available services.
Supporting individuals to access community resources, local services, voluntary sector support, and informal networks to maximise independence.
Working with adults in crisis to help stabilise situations and identify sustainable support solutions.
Supporting adults with long-term care and support needs through strengths-based assessment and intervention.
Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
Responding to safeguarding concerns and undertaking safeguarding enquiries in line with the Care Act 2014 and local safeguarding procedures.
Assessing and managing risks appropriately, balancing safety, independence, and individual choice.
Supporting Continuing Healthcare (CHC) processes and contributing to multidisciplinary assessments where required.
Arranging Direct Payments and supporting individuals to maximise choice and control over their care arrangements.
Developing person-centred Support Plans in partnership with service users and carers that reflect outcomes, strengths, and identified risks.
Working collaboratively with families, community organisations, health services, providers, and partner agencies to achieve coordinated support.
Maintaining accurate, timely, and legally compliant records, reports, assessments, and case documentation.
Participating in duty functions and contributing to the smooth operation of the Locality Team.
Providing guidance, support, or supervisory assistance to colleagues were directed by the Team Leader.
