We are seeking an experienced
Palliative Care Administrative Coordinator to lead the administrative operations of our Palliative Care and Dementia GUIDE programs. This role oversees program administrative staff, manages key operational processes, and partners closely with clinical leadership to ensure an exceptional experience for patients, caregivers, providers, and referral partners.
Reporting to the Chief Medical Officer
, the Palliative Care Administrative Coordinator works collaboratively with the Clinical Manager, Nurse Care Coordinator, interdisciplinary clinical teams, and Patient Access Services to support efficient, high-quality program operations.
What You'll Do
Leadership & Team Support
Supervise and mentor the administrative staff supporting the Palliative Care and Dementia GUIDE programs.
Foster a culture of collaboration, accountability, responsiveness, and service excellence.
Support onboarding, training, and ongoing development of administrative team members.
Program Administration & Operations
Oversee daily administrative operations for the Palliative Care and Dementia GUIDE programs.
Manage program scheduling, documentation workflows, reporting, data tracking, and operational processes.
Maintain accurate program records, policies, procedures, and department resources.
Support quality initiatives and program growth efforts.
Clinical & Patient Care Support
Partner closely with the Clinical Manager, Nurse Care Coordinator, physicians, advanced practice providers, nurses, social workers, and other interdisciplinary team members.
Collaborate with Patient Access Services to support a smooth, timely referral and intake process.
Coordinate communication and administrative workflows that promote seamless patient and caregiver experiences.
Ensure clinical teams have the administrative support needed to focus on delivering exceptional care.
Operational Excellence
Identify and implement process improvements that enhance efficiency and service delivery.
Monitor program performance metrics and assist with operational reporting.
Support compliance with organizational policies, regulatory requirements, and program standards.
Assist with implementation of new systems, workflows, and operational initiatives.
What You Bring
Minimum of five years of experience in healthcare administration, medical practice operations, care coordination, or a related healthcare setting.
Experience coordinating activities in a multidisciplinary healthcare environment.
Previous experience supervising or leading administrative staff preferred.
Strong organizational, project management, and time-management skills.
Excellent communication, interpersonal, and problem-solving abilities.
Ability to manage multiple priorities in a fast-paced environment.
Proficiency with Microsoft Office applications and healthcare information systems.
Ability to maintain confidentiality and exercise sound judgment when handling sensitive information.
Associate or bachelor's degree preferred.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Care Dimensions also offers a comprehensive benefits package – including employer-sponsored health insurance, paid time-off, tuition reimbursement, student loan paydown, supplemental insurance options and more – which adds to
the company’s generous total rewards package.
Pay Range
$29 — $33 USD
Benefits are offered to employees that are scheduled to work 20+ hours/week, which include a generous earned time (vacation days) program, tuition reimbursement, scholarship programs, student loan paydown program, two retirement plans, in addition to medical/dental/vision/life/disability insurance, and so much more!
