Job Title: Day Center Aide – Personal Care & Participant Support

About the role: Provide hands-on participant support and assist older adults throughout the day center with mobility, supervision, activities, meals, and appointment escorts to help maintain independence, dignity, and quality of life. Training is available.
Key responsibilities:
Assist participants with daily activities and personal care
Provide participant supervision and safety monitoring
Assist with mobility support, ambulation, and transfers
Support recreational and social activities
Assist with meal service and feeding support
Escort participants to appointments as needed
Encourage participant independence, dignity, comfort, and engagement
Communicate participant concerns to the interdisciplinary care team
Assist with documentation and reporting
Support a safe and welcoming day center environment
Required qualifications:
High School Diploma or GED
Personal Care Certification or ability to meet training requirements
Home Health Aide (HHA) Certification accepted
Ability to complete competency training before working independently
Ability to work on-site Monday–Friday, 8:00 AM–4:30 PM
Reliable transportation
Strong communication skills; compassion and patience when working with older adults
Ability to maintain confidentiality
Ability to perform physical job duties and assist with supervision, mobility, ambulation, transfers, recreation, meal support, and escorting participants
Ability to pass medical clearance requirements
