POSITION SUMMARY
Provides direct patient care and related support to medically fragile clients and their families under the supervision of a Registered Professional Nurse or supervising licensed therapist, and within the realm of accepted standards of practice for a Licensed Practical Nurse as stated in the Nurse Practice Act for the state of practice.
DUTIES AND RESPONSIBILITIES
Ongoing interdisciplinary assessment of the patient
Implementation and evaluation of the plan of care in partnership with the patient, representative (if any) and caregiver(s)
Providing services that are ordered by the physician as indicated in the plan of care
Patient, caregiver, and family counseling
Patient and caregiver education
Preparing clinical notes
Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care
Coordination with known external agencies providing care and services to the same patient
Participation in the agency’s QAPI program
Participation in agency-sponsored in-service training
Provides assistance in the implementation of the plan of care
Assuring that patient needs are continually assessed
Ensures delivery of services is maintained in accordance with physician orders and with client consent
Observes, records and reports reaction to treatment and any changes in patient condition to appropriate personnel and/or physician
Follows established departmental policies, procedures, and objectives
Practices safety, environmental, and/or infection control measures
Performs miscellaneous job-related duties as assigned by Clinical Manager/Administrator
Participation in regular on-call rotation as directed by the Administrator
Requirements
JOB REQUIREMENTS
Reliable transportation and working telephone as required by the nature of the position.
Age: Must be a minimum of 21 years of age
Education/Licensure/Experience
Graduation from an approved School of Practical/Vocational Nursing and current state Licensed Practical Nurse license.
Prefer one (1) year work experience as a practical/vocational nurse in a hospital. Prefer six (6) months Home Health experience.
