POSITION SUMMARY
Provides evaluation, direction and application of skilled services to carry out the plan of care and treatment in the homes of Phoenix Home Care clients and within the realm of accepted standards of practice for a Registered Professional Nurse as stated in the nurse practice act.
DUTIES AND RESPONSIBILITIES
Completing comprehensive and OASIS assessments.
Ongoing interdisciplinary assessment of the patient.
Development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
