Social Worker - Community Team

Liquid Personnel is recruiting an Adult’s

Social Worker to join its client’s Community team, based in Wakefield.

What will your responsibilities be?

Undertake comprehensive assessments of individuals’ social care needs, considering their personal circumstances, strengths, risks, and support networks.

Develop, implement, and review person-centred care and support plans that promote independence, wellbeing, choice, and control.

Assess eligibility for social care services in accordance with relevant legislation, policies, and local authority procedures.

Identify and manage risks, including safeguarding concerns, self-neglect, abuse, neglect, and situations where individuals may be vulnerable.

Coordinate and arrange appropriate care and support services, working collaboratively with individuals, families, carers, and external agencies.

Work in partnership with health professionals, multidisciplinary teams, housing services, and voluntary sector organisations to achieve positive outcomes.

Promote strengths-based and preventative approaches, supporting individuals to maintain independence and access community resources.

Undertake reviews of care and support arrangements to ensure they remain appropriate, effective, and responsive to changing needs.

Provide information, advice, and guidance to individuals and their carers regarding available services, support options, and their rights.

Maintain accurate, timely, and confidential case records, assessments, care plans, and reports in accordance with organisational and professional standards.

Respond appropriately to urgent referrals and complex situations, prioritising cases according to assessed need, risk, and statutory responsibilities.

Contribute to safeguarding enquiries, multidisciplinary meetings, and joint working arrangements to promote the safety and wellbeing of adults at risk.

Support carers by identifying their needs, undertaking carers’ assessments where appropriate, and facilitating access to relevant support services.

Ensure all practice complies with relevant legislation, statutory guidance, organisational policies, and professional standards, including the Social Work England professional standards.

Participate in supervision, reflective practice, training, and continuing professional development to maintain and improve the quality of social work practice.

Qualifications and Experience:

• To be successful in this role you must have; Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
