Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and improve outcomes.
Developing, implementing, and reviewing person-centred Care and Support Plans that maximise independence, wellbeing, and community inclusion.
Completing Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
Undertaking safeguarding enquiries and responding appropriately to concerns relating to abuse, neglect, exploitation, and self-neglect.
Assessing and managing complex risks whilst promoting positive risk-taking, independence, and informed decision-making.
Supporting individuals to access community resources, direct payments, supported living services, and other appropriate support arrangements.
Working collaboratively with health professionals, housing providers, care providers, carers, advocacy services, and partner agencies to achieve positive outcomes.
Coordinating complex care and support arrangements and monitoring the effectiveness of services provided.
Supporting individuals through periods of crisis, transition, deterioration in health, or changes to support arrangements.
Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences.
Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.
Promoting strengths-based practice and empowering individuals to exercise choice, control, and independence in their daily lives.
