Liquid Personnel is recruiting an Adult’s
Social Worker to join its client’s Discharge to Assess team, based in Manchester.
What will your responsibilities be?
Undertake timely assessments of adults with health and social care needs to support safe and effective discharge from hospital.
Work collaboratively with hospital teams, NHS colleagues, community services, reablement teams and care providers to facilitate timely discharge.
Apply the Discharge to Assess (D2A) model, supporting individuals to leave hospital when medically appropriate and complete longer-term assessments in the most suitable setting.
Assess individuals' immediate care and support needs, identifying risks, strengths and available support networks.
Develop interim care and support plans that promote safety, independence, recovery and wellbeing following discharge.
Apply the Care Act 2014, Mental Capacity Act 2005 and relevant safeguarding legislation and procedures.
Undertake mental capacity assessments and support best-interest decision-making where individuals may lack capacity to make specific decisions.
Identify and respond to safeguarding concerns, ensuring appropriate risk management and protective measures are in place.
Engage with individuals, families and carers to establish their wishes, preferences and support needs throughout the discharge process.
Explore alternatives to long-term residential care, promoting reablement, rehabilitation, intermediate care and community-based support wherever appropriate.
Coordinate appropriate care packages, equipment, adaptations and community services to facilitate safe transitions from hospital.
Contribute to multidisciplinary meetings, discharge planning discussions and complex case reviews to support effective decision-making.
Undertake follow-up assessments and reviews following discharge to determine longer-term care and support needs.
Work proactively to prevent unnecessary hospital delays, reduce avoidable readmissions and promote positive outcomes for individuals.
Liaise with commissioned care providers, housing services, voluntary organisations and other agencies to ensure continuity of care.
Manage a complex and fast-paced caseload, prioritising work according to urgency, risk and discharge requirements.
Maintain accurate case records, complete assessments and prepare reports in accordance with statutory requirements and organisational policies.
Promote person-centred practice, dignity, choice, independence and least restrictive care arrangements throughout the discharge process.
Ensure individuals and carers receive clear information, advice and guidance regarding available support and ongoing care arrangements.
Maintain professional accountability, participate in supervision and reflective practice, and contribute to service development and quality improvement.
We are unable to support or accept applications from candidates who are residents within the Red or Amber list of the Code of practice for the international recruitment of health and social care personnel in England, based on the World Health Organisation (WHO) Workforce Support and Safeguard List.
