Job Duties/Responsibilities Lead the assessment process for your assigned patients, completing comprehensive psychosocial histories and proactively identifying and resolving any gaps in documentation Collaborate with patients’ support systems and interdisciplinary team members to ensure a well-rounded understanding of each individual’s needs Develop timely, individualized treatment plans with 72 hours of admission, setting a strong foundation of care Create personalized crisis plans for every patient, ensuring preparedness and
